At Born Strong Martial Arts, we are committed to making our programs accessible to families who need it most, regardless of financial circumstances. To support this mission, we offer a reduced membership fee for those who qualify based on income.

Purpose of the Form: This Income Qualification Form is designed to help us determine your eligibility for membership and possibly a reduced membership fee. By providing the necessary information, you enable us to offer financial assistance to families who need it most.

Who Should Complete This Form: If you are interested in enrolling with Born Strong Martial Arts, please complete this form for consideration. 

Information Required:

• Household income details
• Number of dependents
• Proof of income (e.g., recent pay stubs, tax returns, or other relevant documents)

Confidentiality: All information provided will be kept strictly confidential and used solely for the purpose of determining eligibility for membership and/or financial assistance.
